In a 66-year-old male (CHA2DS2-VASc score=3) referred for catheter ablation of persistent atrial fibrillation, transesophageal echocardiography revealed a large left atrial appendage (LAA) thrombus (Figure A; Movie I in the Data Supplement), despite continuous treatment with rivaroxaban 20 mg daily.
